Student Proctor Agreement Form

This form constitutes the agreement to administer tests at Greenville Technical College.

Student Responsibility:

· Call 864-250-8020 or email DistEdTestCtr@gvltec.edu for appointment within 24 hours of testing date, if not scheduled by your university.

· Visit website for testing hours www.college-online.com.

· Provide postage-paid return envelope for testing materials (if not provided by university).

• Present photo ID
• No cell phones, beepers, or electronic devices.
• No children on Greenville Technical College campus including testing center area
• No pencils, pens, or paper.
• No book bags, backpacks, pocket books, or purses.

Testing Center Responsibility:

· Administer tests/exams based upon established criteria to safeguard and ensure the integrity of testing materials with focus on the following objectives:

1. Administer tests/exams only in the secure testing center.

2. Store tests/exams until distribution to the student.

3. Ensure that no tests/exams are photocopied.

· Test will be administered according to the directions indicated on the cover letter and/or proctor agreement letter.

• Pencils, pens, and paper are provided for student convenience.
